- Submit the supporting documents in a sealed envelope to your Property Specialist:
1. PROOF OF IDENTIFICATION (VALID ID)
a. Any two (2) valid government IDs
b. Company ID (if SERP account)
2. PROOF OF BANK ACCOUNT NUMBER DETAILS (any of the ff. copies)
a. Bank Statement
b. Validated transaction slip
c. Passbook showing the account number
d. Check with account holder’s name
